Dental EFT Enrollment Forms

Change Healthcare has developed processes to allow payers and providers to electronically exchange sensitive medical claims, patient eligibility, ERA, and EFT payment information.

The Change Healthcare EFT service allows providers to have payments from EFT participating payers deposited electronically into their bank accounts at no cost. Participation in EFT is limited to those providers who have a Dental Connect (DC) account.
